COVID-19 Vaccine FNQ School Program

​​​Commencing Monday 1 November, Western Cape College is partnering with the Pharmacy Guild and Sam from Live Life Pharmacy Weipa to participate in the COVID Vaccine FNQ school program. ​​​​​

The Program is open to students aged 12 years and over who wish to be vaccinated, and whose parent, legal guardian or authorised person has provided consent for them to receive the Moderna (Spikevax) vaccine.   Vaccination is not compulsory but is highly recommended by the Commonwealth Chief Medical Officer and Queensland's Chief Health Officer. 

All vaccinations will be administered by Sam from Weipa Live Life Pharmacy.   Please refer to the attached letter for parents and guardians, as well as the Flyer on the Moderna Spikevax.  If you wish your student to participate, each student needs legal consent prior to vaccination, regardless of their age above 12 years, and each must have their own unique consent form. Families with more than one child at the school will need to complete separate forms for each child.  Completed forms should be returned to the administration office so that we can coordinate with the pharmacy.
